Anyway, I picked up the wheel from the machine shop today. The guy milled out the lug holes, and made new recessed areas for the larger washers. It came out nice:
Took the wheel back to Discount Tire to have it mounted up. The installers said it was a bit different from the others (duh) and instead of putting it up front, they put it in the back. Made no difference, as the wheel weights still hit on the caliber bolt. I don't know what they did to make it work, but all four are now mounted up:

When i took em in to get mounted up i noticed one looked a little different, the "kidney" shaped holes were contoured slightly differently and the valve stem was further outboard.
But most importantly to me i also noticed this particular wheel had an 1/8th inch less "meat" around the center hole where the lug holes are, turns out this 5th wheel was a Western brand slot.
So needless to say i had them mount up the four Ansen slots and used the Western slot as my spare, but i had almost overlooked it entirely.
