Multiple cuts on a piece of box tube might work? Cut one side off to make it a U, then grind the one side down that's over the two bolts for appearance, and then grind the angles into the front and back for the same. Then drill the holes to bolt it together. My 2 cents. That is, if nobody has one for ya.

Vendor below shows to have one, others may have it too. Interestingly, the picture in the OP shows the bracket installed upside down, compared to the parts manual?! Maybe it's homemade?

I'm guessing the pedal side hole was "egged out" and sloppy or stripped? Mine was. I made a new one by cutting & welding two pieces of angle iron. I also improved the alignment of the pivot arrangement by locating the hole more accurately. I had the advantage of no cab mounted so access for fabbing was easy.
