Spare tire holder, biggest spare?

What's the size (biggest) tire will fit in the spare tire holder with a 4" exhaust?

A worn 35 will fit,but it makes the exhaust rub on the shock. I also does not sit perfectly up against the framework of the tow hitch

Depends on how your exhaust is routed.

I have 315's (35's) and the spare just barely squeezes in there and my exhaust is not in the way.

The reliability of that spare tire holder depends on a flip of a coin. Mine puked the very first time I lowered a 285/75/R16. They have plastic gears in them.

I fit a 33" tire along with a 5" exhaust. In my case I make sure to slide the tire away from the exhaust before giving the pulley its final snug.

I've owned my pickup for 5 years, always had big tire on the spare, never had an issue with mine.
