Previous generation of Powerstroke engines allhad the element in the block, The option was just to get the cord. The 6.7L is the first powerstroke to not come standard with the element installed. ( it is standard in about 14 cold weather states). I ordered my truck with out the block heater, assuming if I needed it I could just buy the cord like my previous 4 Superduty trucks. Not so.
The install involved pulling the plug from the block and inserting the new element and then attaching and running the cord. There will be some fluid loss when pull the plug. I've heard around high $300 for the going rate, but have not priced it out myself.
