Defrost is the default when there is no vacuum. If 4x4 is working properly, then you need to look at the lines going to the HVAC under the dash, and you need to rule out your vacuum pump is working...

There are little clips in the door and the trim fits over these clips and holds it on, unless the holes in the trim are ripped you can get them back on. It's not easy, takes a little patience.

The fan doesn't come on until 215 or 217 on full speed. So it does look worrisome to be over 205 idling, sitting in traffic, but rest assured when she does come on, even at idle, it cools things...
