There's no way someone broke a functioning steering box by cranking hard on a steering wheel. Like Brad said, you need to check the rag joint before assuming the gear box is dead.

Look at the rear axle ratio, as others have said. My guess is you could see a big improvement by switching to some 4.10s.

I saw 17 mpg highway with a 4x4 SCSB 4.9 E4OD with 33s and 3.55s, but I drive VERY conservatively. I'd think you could get high 18s if you drive in a similar manner with a properly tuned truck.
