Anyhow, I'm thinking your ICP may be low while cranking. With a little more RPM when you pop start it, the ICP comes up just high enough...

You may or may not get a code for low cranking ICP. The PCM doesn't directly monitor the IPR; it only knows the commanded duty cycle. Try disconnecting the wiring connector from the ICP sensor next...

The symptoms are in line with a high pressure oil system leak. Possibly IPR orings or injector o-rings. Do you have a scan tool you can use to watch IPR duty cycle and ICP?

You've misunderstood. The later trucks are supposed to have the long thermostat. Test your thermostat. Put it in a pan of water and heat it on the stove. It should open just before the water starts...

On another note, people have made fixtures to pressure test the cooler. That could be a good way to determine whether it's really leaking through the tube wall, or if something else is going on.

If it is actually leaking through the wall, I think if I were in your situation I'd order a new one. That's one of those parts that will almost definitely leave you stranded if it fails.

When it's cold outside, it should idle up a couple minutes after starting. Application of the throttle, brake or clutch should kick the idle back down to the normal 675 RPMs or so.
