If it's a recirculating heater you need one cable and the switch. If it's a fresh air heater you need three cables and the switch. A fresh air heater will have an opening on the side to draw air in from the outside via a duct.

C&G Ford Parts has the cable and the **** in a kit. It's part number BAAA-18518-R . (53-55) ($30) Just FYI the Fresh Air Heater ****/Cable Kit is P/N BAAA-18518-F ($65)

They sell the 6 volt switch as well P/N 1C-18578-A ($70) (51-55) or the 56 12 volt switch (which would be what you want if you have converted to 12 volt it is P/N 1A-18578-A ($27)

Here's the link to C&G :

Heres a line art drawing of your heater and the cable connections:

Someone just converted it to a recirculating heater. That opening used to be controlled by a door that allowed you to suck air into the heater either from within the cab or from outside.

Since the duct is missing it has no connection to the outside, and they blocked that inlet off. If you can find the duct (and I would think it wouldn't bee too tough (maybe) you could open it up and hook it up to the outside. You do have the louvered opening on the side of your cowl right?

If you don't want to fool with it, just leave it blocked off, install it as a recirc heater and only buy the two controls - for fan and defrost/heat.

Your truck either had the recirculating heater originally or no heater at all. If your louver is blocked off it was that way from the factory.

If you are going to use the heater you have as a recirc heater why not buy a recirc heater on ebay. I see them all the time. I think they are a lot prettier! Look at mine:

You definately need the cable kit - the P/N for it is in RED above.

And you will need a switch. If your truck is still 6 volt positive ground you will need the switch highlighted in PURPLE above.

If your truck has been converted to 12 volt negative ground you will need the switch highlighted in GREEN above.

The number for C&G is on the link provided!

PS Part numbers should be th esame for Dennis Carpenter if you prefere them. I like C&G much better. Stya away from LMC, they are kinda screwed up lately.
