I've been boning up on all this stuff. My truck is going to have a divorced transfer case with 4x4, so I've got two drivetrain members, two axles, and three driveshafts to mount and phase. It's going to be interesting to say the least. Have a look at John Niolon's great driveline phasing article and cypher on it for awhile. It's clear and well written and should answer all your questions. It can be found here:

Also remember that you've got to consider total driveline angle when making your calculations. That means figuring your side-to-side offset and adding it to your vertical offset to get the total. If you want a little heavier reading, check this Dana document out:
