There are quire a few guys running it now, but it really is a "time will tell" situation. It's ultimately up to you, but I think I'd swap out the green junk at least for gold if I had it. Being free of the constant maintenance on that stuff would be worth it to me.

I went over to the Cat dealer the other day -- it'll cost me about $90 in coolant and about $20 in distilled to do it. Plus I want change that pesky hose, and maybe re-do the 'stat housing just to make sure I don't have any issues later. Maybe even the lower hose housing O-ring too. If I put this stuff in, I don't want to have do it again later.
Don't forget the belt while you're in there! Fyi, there's two different upper hoses type, one goes through the belt the other around it. I stuck with what I had and went through the belt. Only issue there is if you need to change the belt the hose needs to come off. If you go with the hose that goes around the belt, if the belt ever broke it would hit and maybe cut the hose. Just figured I'd add that info if you don't already know.
Great pics, not only the dropout you have to worry about, it's the rust from the upper and lower stamped steel intake and outlet coolant housings too, this is what my upper looked like after my T-stat changeout , the lower looks grungy also, that will be changed out eventually. Dieselsites aluminum housing on the left, stock stamped steel on the right. CAT red ELC user here with the D-S coolant filter system installed. I'll be pulling my coolant filter soon to see what I caught, been about 6 months approx.
Eeww. Mine didn't look anything like that. I've never seen any of that silicate, either...and I did a full flush on my '99 last year.

Looks like ELC to me! And with a 2002, he had semi-ELC in there to start with. Imagine how much silicate drop-out is in a green coolant motor! I need to get this done. I've been putting off the coolant change for a while now.....
EDIT: It's NOT "casting sand". It's drop-out from coolant with SCA in it. Gold has a little SCA, green is full of it. Here's what it looks like:

I know exactly what your talking about. Stowers(Cat dealer), and Fleet Pride were the only places that knew anything about ELC and SCA for that matter.

i went to my international dealer and got the concentrated stuff. Delo makes it and youll need 4 gallon of concentrate to get the job done. its actually good for 800,000 miles and can go to 1,000,000 if you add the booster to it......
Dang it. When I switched mine over this past spring, I didn't know about the lower hose housing being replaceable, and didn't nbotice it while doing all the work, either. I replaced both upper and lower hoses, belt, stat, stat housing, and installed the Dieselsite filter to boot. I though the only issue I was going to be facing was the heater hoses down the road, but I'm going to have to investigate the condition of my lower housing.
Joe (and anyone else), when you do the new upper hose around hte belt, Home Depot now carries some super wide (like 1/2") cable ties that work wonders to snug the hose up to the alternator frame so that it can't flex out and rub the belt. Good cheap insurance for the hose.

The trick there is to add the booster at the recommended mileage point. Adding it on teh front end of the swap won't do you any good. The CAT ELC (which is really Shell's Rotella ELC) has the same kind of option to boost the coolant part way through its life. Just check it about every 50,000 miles for proper pH levels.
Thanks, Pete. I was just going to use a hose clamp. Not all water pumps have the replaceable lower housing. Some are one piece with an extension molded in place with the rest of pump:
