im not sure what brand they are. you might try just looking at different wheel manufacturers and seeing if they have any that match. and as far as center caps, go to like napa or an auto parts store or a shop that specializes in or sells trailers, and you can find some pretty nice generic chrome center caps. take a wheel and make sure they fit, unless you are looking for a branded or model specific cap.
as in when they milled/machined them. Not a flaw during the construction, but a flaw in the design. If you put to much torque on them, you will literally break the wheel because there is not enough metal to take all the torque. The wheel will break at the weakest point, where the circles meet. You learn things like this the hard way.

It shouldn't matter. These usually push in from the back. You could get them plain or with the three spoke knock-offs. The wheel centers are usually the same diameter unless they are 4wd.

Well, the link above for the gambler caps says that they don't push in from the back. Their installed from the front.
