New brakes AND new rotors
My f350 now has 145,000 miles and the brakes are getting close to being toast. At this point rotors are all good and smooth with no warping. Should I just replace the pads or should I consider pads and rotors?
What about the high performance rotors with holes and slots and pad packages I see on ebay?

Drilled/slotted rotors are a Fad. The only benefit to them are the directional vanes which help them cool off. Quality pads don't degas, so there is no need to give them a path for it. I have power stops on my truck currently. My next change I am putting Performance friction 2pc rotors on.

If you truck is older than 2004, just get the new rotors. when you turn them, they might still be in spec, but they wont for long as they tend to warp easier with less material. Depending on how much of hte 145k miles are on that set, you might be very close to replacing anyways. If you are older than 2004, you might look up the cryo and hawk pads, they tend to survive better. For 2005 or newer, the standard discs are good, I would recomend NAPA Gold, too. That is what I used on my 2000 and they did well for the 20k miles I still had it for.

I am suprised you have that kind o mileage on originals. Must not be a lot of hill country, foot wire tied to the go peddle. I would go with normal replacements, but look into what your options are, for this light a use, you must not pull a lot or haul a lot of weight? I would recemend NAPA Gold, just because they tend to stand up well. On your make and model year, its not uncommon to do front rotors and pads at more like 50 60k miles and rears at 60 - 80k.

That's spectacular! I have 129,000 miles on my original pads/rotors on my 1998 Mustang GT. That's unheard of on a Super Duty!

The heavy service 2pc rotors are flat. I've held one, but my rep didn't have a pair he could leave me. I think they have some slotted options, but the heavy service (fleet type) rotors are flat.

Your looking at the erataspeed catalog. Baer does show decelarotors for the 99-04 2wd and 1999 4wd.
Brembo show pns for 99-04 4wd. PN 27245. And another pn for 2005.
DBA had a couple of part numbers too. I don't know if they brought them into the country yet though. Last I looked the 4000 series hadn't come over.

I just did mine. 98,000 and change. Almost positive everything was original. Well I just put some cheapo pads on for now. I am planning on rotors and pads from napa the best ones they sell. The rear rotors are like $96. pads are somewere in the $60 range I havn't priced fronts yet.
Just a quick question were would I get emerg. brake pads Do you think Napa will have them or are they like a dealer thing?
