Output Shaft Bearing

While I am enjoying my 25mpg, the dealer had my tranny and transfer case out for the 2nd time in 14,000 miles to replace the outputshaft bearing. There was a slight squeek when cold the last time and this time the squeek was all the time. When they tore into it they found that the bearing was pitted both times. I told them that I want them to find what is causing this problem. They have changed the torque converter and flushed the rest of the tranny and changed the bearing and are putting it back together now.
I am just trying to see if anyone else has had this issue and what was cause of it. Any ideas at all.

They said that the torque converter was bad and putting trash(metal) into the fluid. The replaced the TC and overhauled the tranny. They also performed at TSB on the tranny and did a reflash to fix the shift strategy. We shall see how this goes.
