Escape throws P0191 code, help

I need to know what would cause this thing to throw the code.

Here's the history. This is my buddies girlfriends vehicle. She took it to some podunk service station for oil changes. That's all they changed. With 80k miles on it the air and fuel filter were never replaced. The rig tried to die on the road. Replaced the filters and it ran. About 2000 miles later the fuel filter clogged again. Today it died again. Threw a new filter in and it didn't fix it this time.

So my question is, Is it the fuel pump? This is where I lay my money since the pump had to work so hard to get fuel to the engine.
