It all depends on the custom tuning, if you datalog it and how good its tuned now. These vehicles vary quite a bit in performance and economy right from the factory. Sometimes there's a lot to be gained, sometimes now.
With proper datalogging a custom tune can be made which will squeeze more power and mileage out of it. On the 5.4 3-valves there's lots of power and mileage available even with 87 octane. On your 2-valve not nearly so much. Keep in mind if your truck gained 1 mpg its going to take the average driver more than a year to break even on the $375-$450 investment on a custom tuner. Your primary motivations for a tuner should be power and driveability, with mileage a secondary benefit if you're lucky.
