First of all, what tranny do you have. I assume that because you have the 4.6 (like my truck) that you have the 4r70w. Where did you get the shift improver, what did it do for the tranny, and how much did it cost you. I have yet to find a shift improver for my truck so if you have the 4r70w, It'll really make my day
Also, what type of system are you running for the duals?

K/N Filter charger?
I put a K&N FIPK on a 2002 F150 4.6L auto supercab with the 3.08 axle. I don't seem to get higher gas mileage - the best is usually around 19-20 on the highway, which is the EPA rating. However, I don't seem to get really low mileage anymore - I used to get 15 or 16 MPG around town or mixed driving, now it never seems to go below 17. I also tend to get the higher numbers more consistently.
If there's a headwind or hills, the mileage drops 1-2 MPG - that's probably because of the diff ratio. I don't recommend the 3.08 axle.
I didn't really get more power, but my truck runs such low RPM's that it's not really in the range for a power increase. I usually run about 1900 RPM at 75 MPH. And yes, it downshifts a lot.
What you really notice is the throttle response - the truck really picks up when I step on it. It's a lot more fun to drive, and that made the kit worth $228.
I just my kit in a few months ago - the kit includes a modification in case you get whistling. It looked like something K&N put in after the original design. I didn't put the mod on, and mine doesn't whistle ever. But when I get above 2500 RPM, I get a great roaring sound out of the engine.
