riggz: Great writeup and pics.. Glad someone finally did it.
Keep in mind there are tapered and non-tapered blocks. I'd like to see you add a note to the part numbers indicating that there are deffinitely two different blocks and you need to determine which ones your truck needs. As fas as I know, SC SHort box and shorter trucks use tapers. CC short box and longer use non-tapered. But I'm not 100% sure of that.

Here's what I've come up with to add:
If you have one of the following:
Crew Cab Short Bed (156" Wheelbase)

Blocks: F81Z5598DA x 2
U-Bolts: N803770S436 x 4
Nuts: N620485S441 x 8 (You can reuse the old nuts if you wish.)

Riggz, Great write-up. I did the same swap on my 2003 SD 6.0 CC SB. I used the same part numbers you list. The only thing I would question is the part number for the U-bolts when using the tapered blocks. By mistake I ended up with the 5C3Z5705AA U-bolts, they had to large of a bend (to wide) to fit my axle housing and would not easily slide though the holes in the top plate. They are the correct length although. I'm not sure why the U-bolts would have to be different between the tapered blocks and the flat blocks. I believe the block length is almost the same except for the few degrees of taper. It could be 5C3Z5705AA U-bolts are for the 2005-7 years, with the bigger brakes and load capacity. They may have increased the axle housing diameter. Maybe some else could help out here????
One other number I used for the carrier bearing, was for the Ford shim kit comes with three shims 1/4" each, EOTZ4A209A. I used one of the shims and reused my two bolts with locktite. If you need to use more than one you will need longer bolts. I checked with my local Ford dealer and he said the 250 and 350 use the same bolt. It did not make sense but I did not pursue.
