The jack should be on your passengerside inner fender, and the lugnut wrench should be on your driverside inner fender.

95 PSD 250 the jack and lug wrench were on the right inner fender with the lug wrench tucked into the gap between the fender and inner fender. The jack handle was across the top of the radiator support.

Same here... seems like every truck I've ever bought was missing either the jack, or the handle, or the lug wrench. And even if they were there, most of the time they were either did'nt work at all or were'nt working right. My solution was to get a hydraulic jack and a 4 way lug wrench for my '88 F150. The jack is small enough to stow behind the seat, and the wrench goes under it. I'm going to get the same thing for my Bronco... just have'nt gotten around to it yet.

I guess I never knew there was a handle, because alls I have is a jack and the lugnut wrench works as a jack handle, but I will never try to use that mickey mouse, I would rather just use my Hi-lift

You're just as well off without it. The stock jack sucks just like most others. Do as someone else said and go buy a cheapo hydraulic and be done with it. The first time you use it you will make up the cost in time savings.
