There should be a phillips head metal screw at the bottom of the sunken area on the arm rest and one in a recessed hole under the vent window. The remaining fasteners holding the door panel on are the ribbed plastic push-in type. Be sure to get under the head of the fastener with a flat bladed screw driver and pry them out. The fasteners are available at auto parts stores in case you break some...like I always do.

It might not hurt to spend a couple of bucks and get a door panel removal tool.It looks like a large flat two pronged fork, and will make removal a heck of a lot easier. I would also recommend picking up a box of the panel retainers due to sometimes damaging the originals upon removal. Sometimes you can reuse them, but experience has taught me to go ahead and replace them anyway. One other thing, there some screws along the bottom of the door panel. Sometimes they are hard to see due to the carpeted panel bottom.
