Hesitation when cold

I am using my father in laws 90 ranger with 310,000 miles on the original motor. when i fist start it i have to make sure that i keep the throttle closed or open more than a 1/4 of the way. once this motor warms up it runs fine. the only work done to the truck since new is 2 clutchs and a trans @110,000 because of poor workmanship @ 90,000 mile maint. what do i need to do to get rid of the hesitation.

I would clean the throttle body first, since it hasn't been cleaned in 310,000 miles, if that doesn't help it I would then look at the Idle air control. Also it wouldn't hurt to pull codes either.

im willing to bet the upper and lower intake manifolds are going to look as bad if not worse than the throttle body. so it wouldnot hurt to clean them too.
