On the trucks (never worked on a van) it looks like the CAC pipe goes into the radiator (this is the intercooler) on the drivers side of the engine and then exits the intercooler on the passenger side with a different pipe before heading back up towards the turbo.
I am sure someone here can chime in on whether or not you have an intercooler in your van.

Slowing down a bit to put all the clues together - p0472 with low power and soot out the exhaust... and a chip.
Step 1: Seriously... pull the chip. One time I was 400 miles from home and chasing my tail because of a bad connection to the chip, and I'm not the only one that has fallen victim to this impish trickster.
If that doesn't clear it up right away, then don't get discouraged - now we're troubleshooting the truck with one less variable. Exhaust leak... bad juju on a turbodiesel. Absolutely zero soot belongs anywhere under the hood, so a soot safari is in order. If you see a five-O-clock shadow on any part under the hood, there's your exhaust leak. When I had leaks, they were so bad that I could put my hand near them at idle and feel the fire breathing dragon's breath.

My '03 E-450 chassis short bus did not have an aftercooler and I do not believe that vans have them. That other radiator that you have up front is most likely your A/C condenser. If you need any parts for that ask me before you buy, I'll let you have them for the price of shipping, what I have left is past due a trip to the scrap yard.

Did you remove the tuner completely or just switch it to a stock setting????
Reason I say is because I have a TS6 and regardless...........sometimes it works, next turn of the key, it's at a bar somewhere. Totally different gal to drive down the road.
(Still haven't figured mine out but the symptoms are like having a blind date)
