Take the tee connection apart, open the radiator, and blast your water through the 2 heater hoses that go to the 2 heater cores. My preference is to remove the lower radiator hose so all of the water drains out after, but we all know how hard that hose is to disconnect.

Yes, correct. To me it looks like a previous owner did a flush by cutting off the crimps and reconnecting everything with hose clamps.

The crimped tee hoses route some coolant for the rear heater.
The section with the hose clamps looks like the heater control valve was removed.
Is there a heater control valve in the lines just outside the view in the pic?
Also, the 2 vacuum hoses in the lower right corner of the pic are looking a bit aged.

Great thread and starting the same project. Seems all my hoses are crimped.
Guess I just need to rip it all apart and re-hose and clamp?

I didn't re-hose everything, but I did rip it all apart. Here's the passenger side of our 2003 5.4L.

Can't really tell what the final 2 connections are right at the firewall.
Looks like you got rid of a majority of the ford connections?

Everything is hooked up exactly the way it came from Ford.
I lengthened/replaced/rerouted all the lines to gain access to the plugs/COPs/IAC.
The 2 connections at the firewall are the heater supply/return.
I cut the one off and used a brass 5/8" barb into a 45 deg elbow to angle it away from the engine.
Here's the PVC line -
