Power steering pulley continues to pop off

96 ford f250
My power steering pump pulley continues to pop off, i know its on all the way. the second time it popped off i was able to pull over and grab it, it was hot as as hell. i dont think the pulley was spinning.

i bought the truck sight unseen, installed a new vacuum pump, power steering pump and s belt. when i got it the previous power steering pump was messed up. The pulley itself was off angle, because the shaft was either bent or... something. i like to do my own work and really dont want to have to take this to a mechanic shop.

any ideas?

The pulley was probably spinning on the shaft and ate it up.
The pulleys are supposed to be changed when removed. They were never meant to be reused although people do my self included. Some never have a issue and some do.
