What has already been said is that your lack of FPW (injector pulse width) could be the FICM or it could be the cam sensor. IMO your crank sensor is OK and probably your PCM is also OK (but can't say for sure). It would be helpful to know the sync information and the three FICM voltages (MPower, LPower, and VPower) at KOEO, and cranking. Without the troubleshooting details you might as well throw a few parts at it - replace the cam sensor and send your FICM to Ed (FICMrepair.com) to be tested and repaired. Better than having it sit there for months.

Have you gone through the set-up screens on the ScanGauge? It has an option for "diesel A" or "diesel B" I believe it should be set to A. If you've been there, check power at the cigar lighter, it the same fuse that supplies power to the pin in the OBD connector under the dash. If that's good, check fuse 22 under the dash, it's labeled "engine controls". Check fuses very closely or measure with a meter, they can be sneaky sometimes, blown but hard to see. Also be sure fuse is going into the clips in the fuse box, it's possible for the blades to go in beside clips instead of between. It will look like fuse is installed properly but it's not making good connection. Small chance there is a problem with the ScanGauge but it's not unusual for it to take a couple minutes to connect if it has been unplugged.
