Sorry should have clarified. I deaf in one ear so the earphone want work. I looking at the ones clip on the sunvisor. The radio I put in has bluethooh but it socks so bad nobody can understand me

I've got a semi-hands free setup, does that count?
By that I mean that I plug my phone into the car speakers(replaced the original radio with a $15 amplifier), with a 3.5MM jack, and my phone calls come out of the speakers. Microphone is the one in my phone, which seems to work well enough.

You misunderstood me. I have the aftermarket stereo in my 91 F250, also with the 7.3. My point is that the cheap stereo in the truck has better Bluetooth quality than my much newer car's factory system.

OK I bought the cheap BOSS audio 7" touchscreen for the monitor for the backup camera and the BLUETOOTH camera ok bluetooth that a joke. It sounds good to me but not the people I try to talk to

The key is the microphone, and esp. your distance from it. Think of how it sounds when you talk to someone on a speakerphone in an office. It has that "tin can" sound because their voice is bouncing around the walls of the office and going into the mic, along with the voice sound going directly into the mic. Same thing with an in-vehicle device with a centrally-located microphone. I couldn't tell you what in particular to look for, but generally, and given your hearing situation, I would shoot for a solution that would put the other caller's voice out through the truck's speakers, and would use a clip-on mic, or the mic portion of a headset, to pick up your voice. I just don't know what's available like that. You may have to fab something.

i have an older version of the Jabra SP700 car bluetooth speaker in my 88, it is about equal to or a little better than the pioneer head unit in the 02 diesel.

I connect my phone to the truck's sound system (aftermarket 10 year old Alpine) via 1/8" jack and use the phone's mic. No complaints on sound quality on either end. Some of it has to do with how good your phone's mic is and if it has background noise cancelation, some with your particular voice. My voice cuts right through the IDI background noise and is clear on the other end. Other people's voices can get lost in the noise.

I purchased a radio unit from Big Lots for somewhere near $10 and it plugs into the phone. It works through your radio frequency (set to ?) and the speakers.
It is called jupiter jack and the connection wasn't bad. Consumer Reports reviewed it 5 years ago and recommended a bluetooth headset instead. It wasnt bad for $10
