My 96 150's 351W had one not two. But it does also have three O2 sensors as well. Two before the cat (one in each side of the Y pipe, one after the cat)
What else could it be? It has a cat and then something right behind it within 6 inches

Cats tend to quiet-en the sound, removing the muffler and replacing it with a straight pipe section and leaving the tailpipe in place will result in a somewhat louder exhaust. Will it run better ? No. The cat's may be plugged up too, that would deaden the sound as well.
There is a 2 stage cat system on all these trucks, the first section is where the pipes come together from the engine and the second a little further back. On an OBD2 truck there will be O2 sensors before and after the cats, sometimes 3 total and sometimes just 2. The factory cats are very dense and do quiet the engine quite a bit, no aftermarket cat you can buy produces anywhere close to the same quieting even the non high flow versions. If you hollow or remove the second cat the exhaust will be louder but you will also get codes for the second O2 which monitors the downstream cats efficiency, but there are eliminator plugs for that.
