2010 shifts hard once in a while

New to the forum. Recently bought a 2010 Transit Connect with a 125k on it. Bought it from a flower shop, Runs great and has had bunch of service done by the dealer over the years. Including transmission service's.

The other day in stop and go traffic it shifted hard into second gear and the little yellow wrench on the instrument cluster came on. After that one hard shift it ran fine the rest of the way home. Once I stopped and restarted it went out. Check for codes and no codes stored.

Did it again today and the only difference was the little yellow wrench light turned off while I was driving. Again no codes

Sometime it seems like it should shift sooner than it does. I let off on the gas a little and it shifts to the next gear. It always shifts smooth other than the hard shifting a couple of times into second.

My overdrive button on the shifter does not work. I can not turn off the overdrive.
Any thoughts would be appreciated.

Trans & filter needs more changes then " book " says

Your sure the trans fluid & filter is fresh & clean ?

Transmission filter and fluid have been changed recently.

Figured it out. It was the output speed sensor. Got it online for about $45.00. Easy to replace and runs like a champ.
