Just to throw this out there, I had a problem similar to what you are describing in my fiance's 2007 Suzuki Grand Vitara. The problem with hers was a gummed up EGR valve. It sometimes gave a CEL and sometimes it didn't. But what would happen is the EGR would stick and would choke the engine out at idle. The computer trying to control the EGR plunger gives very similar results with a low idle, stalling out, erratic idling. They are almost identical symptoms to to a bad IACV.

My Ranger had a high idle pressure switch in the power steering system. When every the pressure increased it would close the switch telling the computer to up the idle to turn the wheels while sitting still.

I haven't dug into my SD like I have my Ranger, but I am simply sharing past experience. I didn't see anyone in the thread mention EGR and it has caused me a similar sounding problem in the past. I thought I would share and maybe it would help.
As for the pressure switch, you asked how it could cause idle issues. I don't know if the SD has the same set up as a Ranger, but you wanted to know how it was possible. That was one way I know power steering can affect idle.

What if the power steering pump bearing was going bad and ever time i start the truck is causes a drag that causes the idle issue? And i was saying the high pitch squeal i heard this morning could be the power steering maybe? But now that we are talking about it could it cause the idle issue?

Got colder again idle is up then falls to a almost stall condition and then pops back up. I cleaned madd air my new IAC valve throttle body etc still wont go away. Sprayed starter fluid all over the throttle body and vacuum lines did not make any effect on the idle so i assume no leaks? Also my "whistle" is back first thing in the morning when i go to work... It does it for about ten to fifteen seconds and then goes away and does not come back till the next morning very very odd .
