Never, once installed properly they should be good forever.
If they are not pounded down all the way by hand, THEN torque the bolt to 120 inch pounds, they can come loose.
Yes, but those injectors have good, fresh o-rings so I would suspect something else.
Bill, did you remove the chip and see if there was a difference?
I'm not saying that's your issue, but I would try some good tuning for your exact set-up before going any further. If that issue persists with tuning, then I would suspect an injector issue. Try to narrow down the injector, have the valve covers off and unplug them one at a time (when it's warm, when it's knocking) and listen for a change in the motor as you unplug each one. This works better than a scan tool. You can do this test now, but if you get a consistent drop across all 8 it's probably something else and not injectors.
FWIW My '96 has Stage-2's with what I consider very good tuning and I swear you could have videoed my truck, it acts just like that. Just the nature of bigger injectors without live tuning, IMO. I know my truck runs well and makes a racket when warm, and I know adjusting the tuning will resolve my issue.
Would also be helpful to monitor ICP and IRP duty cycle when cold and when warm (when it knocks) just to see.
If it was a loose injector from improper install it would do that knock all the time and develop lots of white smoke.
