Just a bit of patience, strong hands and of course replacement parts in hand before beginning. Dorman makes a nice replacement elbow:
If the hose from the PCV to the elbow is in bad shape it can be replaced with almost anything that fits but is also intended for automotive use.
While you're at this double-check the valve cover grommet---it should be available separately too if its bad. I'd use just a dab of dielectric grease around the insert portion to keep it soft and supple, effecting a better seal.
