Could be either.
Jack up the truck with the tire off the ground. Then place a long pry bar under the lifted tire and see if you can lift up on the tire while watching the ball joints. If you see tire movement upwards and ball joint slop with no load on the tire (while pushing the tire upwards), then you know that it is ball joints. It is typically the upper which goes first, but best to replace both at the same time.
