Yes, the spacer is like an adaptor but it adapts two identical lug patterns. 05+ wheels were spaced tighter under the truck when the suspension changed from leaf to coil sprung, so to put them on pre-05s you have to put a spacer in. Just get a good set and not some fleaBay aluminum junkers. Some people talk about how spacers change the centerline of the wheel and put leverage on the bearings yada yada yada, but if you consider that it's changing the backspacing to match the stock wheels and keeping the centerline in roughly the same place it's not as huge a deal. Duallys run spacers to account for the massive offset matching the rear wheels to front, so as long as the spacers aren't moving the tires from where they would otherwise run, it's not the damning no-no some people say it is.
OP, I ran 295/70R18s fine under my truck, but with your backspacing being further out and leaf springs being older and maybe sagged a little, it'll probably be tight. Put them on, and if they rub put a mini-pack into the front.

so what you are saying is I should look for a set of 18 in rims from a 05 or older due to the axle change and the back spacing is different? I only really want the rims the tires on them are worn out.

I thought they changed from the old 8 on 6.5/7.5/whatever it was to the 8 on 170mm in '99 when the SD came out. But I've been wrong before. Even if pre 04 is different, he has to use spacers to fix the offset anyways, and can get a set cut that matches both patterns.
