when I traded my 02 SD in I had 128k and started hearing that ticking sound..I always thought it was injector noise or cam noise. Maybe try putting a can of octane boost or Lucas fuel treatment in and see if it will help clean the injectors and quiet it down. if it does then you know what it is. If you hear the ticking with a quiet thud with it then it could be a tad more serious.

Here are 2 potential causes ;

1- Broken exhaust stud (s)

2-Loosened spark plug

Is it a gas truck? If so, could be cam phaser ticking at hot idle.

The gas engines didn't have cam phasers in 2002.
