Carlite glass ID please

I have this glass and it looks like truck's rear window but can't find any other information on it.

I don't see any numbers on there that tell me anything. Here's the page from the catalog with the rear windows, and I can't see any relationship:

I emailed the Carlite customer service, twice, no reply...

My truck has Carlite glass all the way around. Dad has a spare window in the garage along with the second window that's been in it. I think he worked for a company that made them.

I too have been looking for information on them. I though Carlite glass was factory.
