P0278 one day, nothing the next...

I have a 2005 F350 with EGR Delete and 225,000 miles on it. 3 days ago I had to have the #3 fuel injector replaced. Yesterday, I noticed a rough idle similar to what I felt when I checked codes that had me get the #3 injector replaced. So I checked DTC's and got codes P0278, P0069, P0476 , and P0676. P0676 (Glow Plug #6 circuit failure) has been on a while intermittently, but the other three are new.

Today, I fired her up from dead cold (19 degrees F out all night) and the idle felt fine and there were no codes. There was some black smoke at start up, but there always is on super cold starts and it only lasts a couple seconds.

I'm letting it cool off again, which won't take long since it's friggin' cold out, so I can start her up again and recheck. I just wanted to see what everyone's thoughts were and if anyone has experienced this. Is it possible the injector was clogged and is now unclogged?

Add'l background: Bought truck with 218,000 on it 1 year ago. I don't know what the original owner did to it, but figured out he did EGR delete, head studs, 4" magnaflow, and I believe a new turbo since it's a rebuild from 2003 (nice turbo whistle too). I asked for the original owner's contact info, or email address at least, but of course Ford can't give that out. The tech who replaced #3 injector a couple days ago told me the oil rail has been removed before, but he couldn't tell me why. There are a lot of other mods for towing done to the truck, so he spent a lot upgrading it (air bags, camera, navi...).

The P0278 could have been caused from the glowplug not working on #6. I wouldn't suspect an injector until I'd had a chance to remedy the glowplug problem.
The P0069 means that the MAP, EBP, and BARO aren't agreeing. At KOEO check them to make sure they are within .5 psi of each other.
The P0476 says exhaust pressure control valve range/performance except we don't have one on our trucks. Check that code please.

Thanks npc! Since my garage is tiny, I'm gonna wait until it warms up a little this afternoon to troubleshoot. Maybe it's just chafed wires that I need to fix. I'll post what I find.

Just checked the pressures KOEO:
Baro: 11.75
MAP: 11.60
EBP: 11.89

So those are good. Now onto glowplug troubleshooting....

I have had my injector #4 run rough randomly. Then restart and run totally fine. This happens about once every 6 months and it will never happen more then once. It throws a code and everything. Ive had no ill effects. My mileage is about 16-18 depends if im towing or romping. The weird thing is that injector has been replaced 3 times before. This last ones been in there for 3 or 4 years now.

Thanks for the input. I'm pretty much monitoring now. I haven't found any chaffed wires. I'm a little hesitant to pull the injector to check out the glowplug, but it may come to that. It's just this Christmas and New Year's thing that's delaying me now...
