Well if you unplug both at the same time it is hard to verify which one is bad.
With the MAP something as simple as a clogged or internally frayed line could be closing down and preventing the MAP from reading pressure. If it does anything different (run better/less sluggish) with it unplugged you know that it could be the sensor a block in the line. With the MAP unplugged the PCM reverts to other known values/sensors. Pull the line off and blow/look through it to verify that it isn't blocked.
With the ICP unplugged the truck reverts to a base/default table for values. It impacts your transmission in that your transmission shift points revert to a "default" level. While I've not actually checked I'd surmise that the pressures in the transmission are increased to make sure it shifts for any level of demand.
I bet you would find your hard shifts would be fixed if you plugged back in your ICP sensor, and especially so if you buy a new one. If you find oil in your ICP sensor you know that it needs replacing.

There is a little port on your passengers side intake plenum with a rubber hose connected to it... pull the little rubber hose and clean out the port...
The other end of the little rubber hose is connected to your map sensor... unplug it there too...
Now clean out the hose...
Put back together and see if its better...
If no...
Maybe try a new map.
