So I did a water pump swap today. All went well started the truck sounded great. As soon as I shut the truck of my fan flew into my radiator. Everything was tight or so I thought. Would a bad fan clutch cause it to back off like that?

You didn't get it tight enough.. In theory , it should tighten itself . The variable is you worked on it . It didn't fall off before ...
When you replace your radiator, you can leave the fan and clutch off if you want to. Lots of guys, including me, don't run fans anymore. That's just an option to consider though.
my fan 2 3200 rpm at a sudden lift of the throttle whirled is way throught my rad last summer...i did e-fans...the alternator wont run them though...so they are still on the rad...not working...no loss except to remove them i have to remove the rad.I have done who knows how many wp jobs and never seen it happen.must be a combo of not tight enough and old possibly malfunctioning clutch.
its a gusher when the goes..thats a lot of coolant on the ground real fast!
