1. Maybe bad starter
2. Need ground to engine block or cleaned up existing grounds and like they stated before the heavy gauge wires need to be checked closely.
3. Probably not.
Bad connections are hard to troubleshoot with voltmeter on voltage scale because it will detect voltage, but it won't have current carrying ability caused by a bad connection/wire.
