How long before major repair?
Assuming there have been no major repairs already, how long can you expect to go with a 99-05 F-250 with a 5.4 and automatic past the 200,000 mile mark before you have to expect a major repair? How about with a V10 and everything else the same? I am looking into buying one and I would love a diesel, but they are generally out of my possible price range, so I need to know how much I need to expect to spend on these high mile trucks beyond the initial price.

That is like asking when the next earthquake will come. Might be in 5 minutes or 5 years.
A guy drove a 5.4L in a van to 1.3 Million miles before it blew up. He never did any major repairs and often let the oil changes slide until 50000 miles had passed (Yes, 50k miles). Yes, they were all highway miles (he is a hotshot) but still, 1.3 MILLION miles?

I don't worry about major malfunctions in any of our trucks until 350-400K but with manual transmissions. The 99-03 auto is 50/50 chance of going out by 200K. The 03 and up trans are supposed to be better.
Other than that I wouldn't worry about anything major. Unless you consider wheel bearings kind of thing major?

Earthquakes, Hmm lets see The PNW is over due
Been more than 300 years. So hold on Seattle
the Nisqually was little in comparison to the BIG one
coming and I don't think I would worry about the
wheel bearings when it hits at that point.
But really at 200K miles it really just depends
on past maintenance and some luck. Just like
Guy smokes every day for 60 years no health
problems after his last Dr visit walks out
and gets run over by a GMC truck.

Buy the one you can and try to put some money away for
any needs and wants that may come up.
