What would cause my cab to lean like this?

could be a lot of things . my 53 did this, a 1 inch list to the left side . i looked and looked , replaced the rear mounts and new rubber in the fronts . thought my frame was bent figuring that was the issue as i got 14 inches under the right side front frame rail and 12 1/2 under the left . bought a new frame from boley that i'm still trying to get together . well after the old frame sitting outside of the garage for many moons now with no weight of any kind on it it reads square and true at all four corners . so i'm thinking , despite them looking ok my left front springs were bad . the new rubbers etc. helped for a bit . the old frame had been in a wreck and the very back xmember had an old homemade receiver on it and it had gotten twisted pulling something so i assumed that it was twisted , and apparently was wrong . also now that i'm bleeding green for body work i found that the left front body mount and floor despite looking quite solid were a little soft so .... just don't jump to conclusions yet and investigate it thoroughly so you don't spend money you don't need too like some of us ok doke .

The right wheel is in the furrow. Seriously, get the truck on a known flat hard surface and take measurements. Same tires, left to right, same pressure therein. Measure to the bottom of the springs. Measure top of spring to frame both sides. Measure at various points along the frame, both sides. And then there is sheet metal, fenders, etc that can be sagging. Cab mounts? If they're worn, replace them. It will take some work on your part. And be aware, these frames are designed to flex.

Just put the truck on some jack stands on a flat surface to eliminate the suspension while you check. But I dont think it is suspension because there is a big difference between the cab and the bed. And for the back end of the cab to dive like that, Id expect the bed to do so as well if it was the frame rail. I think mounts, or cab, or some combination.
No way for me to tell from looking, just making a guess.

The cab mounts might look ok but what about the cab floor that they bolt to? I had to totally rebuild this bit on mine to eliminate the sag.
Putting it up on stands under the frame on a flat surface is good advice, take the suspension out of the equation for now and see if the frame is twisted. I have a feeling it could be, that's a lot of lean.

truckeemtnfords suggestion is a very good one. Over the years, I've owned many of these trucks. The springs can cause a massive lean. Mismatched tires, deformations in the cab to mount areas of the floor. Bad bed mount areas, etc. Because the bed wood is gone, the bed is not causing any flex, but it could still sag from rot in the bottom. The frame brackets for the forward cab mount is also a very common failure point of these trucks. If it's cracked, it's not going to be aligned, at all. That much lean, could be all of the above.

I checked it today. The passenger side rear cab mount has collapsed, all the rubber is gone, which caused it to pull up on the other side. I think a set of body mounts will fix it right up.
