OK......I have found some very helpful photos and they confirm how I thought this whole thing goes together....
Next question......Does anyone have a trick for getting the fork to snap in place on the clips on the plate while everything is installed on the truck?
I can get the lower one to snap in place.....but I can tell the top side is not attached.
any thoughts?

I actually got it all fixed and installed today......
it was not easy but I was able to get the clutch fork attached to the clips on the plate.
And as was stated in the beginning of this chain......the bushings were worn or missing on the assembly.
I was able to find any and all parts needed for this at LMC Truck parts.
Thanks again to eveyone that helped.
