I get 9 or 10 pulling my camper at 70mph and it weighs 16K. You have something really wrong. Check for fuel leaks, dirty air filter, fuel filter, etc. Maybe your cat is clogged, get rid of it. More guys will come along.
Depends on speed. Most people don't realize how much more resistance just 5 mph makes. I have 4 wheel drive and a 4 inch lift. If I go 65 (1900 rpm) I get in between 15-17, if I go 70 (2200ish rpm) I get in between 13-15, if I go 75+ ( 25-2600 rpm) I get 12. That's on 285
tires also.
As far as getting better follow the advice posted above. Could be multiple things. Driving habits are the biggest problem in saying I get x amount of mpg vs another guy who gets x amount. Try to drive it without revving past 2k for each shift. I get great fuel milage when I can discipline myself to do it. Air intake and exhaust mods CAN help but bad injectors, leaks, and air restrictions are a good place to start.

Unplug the electrical plug on the MAP sensor and see IC it gets better. The MAP is on the pass. side on top of the a/c box it has a 3 wire plug and a hose on it.
