Do I print it out and then bring it with me to the show?
or do I submit it online somewhere?

Print out the PDF, fill it out and mail it (along with a check for 35 bucks) to the address in the upper right hand corner of the form. If you don't want to send a check there's a paypal link:

Thanks for all the nice comments. The slick 60 was winner from 2010. Last years was a Red 56. The cruise route will head to the NHRA museum in Pomona then to lunch and then roll into Rubys cruise night Ruby's Diner - Whittwood Town Center 10109 Whittwood Dr. Whittier, CA 90603

They will have spaces for us reserved for 40 trucks!
