F250 6.0 Diesel missing-worse after refueling

I have a 2003 F250 6.0 Diesel. It has 49,000 miles on it. I purchased this vehicle new. I have been having issues with it not running well lately. It seems like it always happens after refueling. It has plenty of power but misses and seems to chug at times. I have had it to 2 different mechanics. The one found a hole in the Y pipe on the exhaust so replaced that. Ran great until I put in fuel. Can the fuel cap cause issues like this? Could it be that easy? Don't know what to do. Extremely frustrating. Don't drive much but use it for pulling a 4 horse slant trailer that is loaded with 4 horses. Need the power of the diesel. Love the truck and up until this point have had great luck with it. Got good gas mileage whether pulling or driving with tons of power. Mechanice is puzzled. Don't want to start replacing injectors-mechanic does not think this is the issue.

How can full tank make the engine missing? Beats me.
Such mystery would call for installing fuel gauge and monitoring the pressure to start.

My first thought is/was fuel tank de-lamination...but as far as I know, only the chassis cab trucks with the aft rear axle fuel tank have the steel tanks that de-laminate. What does the primary filter (the one in the HFCM) look like when you change it?

Do you happen to have a way to double check the fuel pressure, especially when it's acting up? And what kind of place are you buying the fuel from? Low or high volume retailer? The '03 and '04 trucks won't always lite up the CEL when they have a stored code. I'm assuming your mechanics checked for those...but wouldn't hurt to double check anyway.

I wouldn't start throwing parts at the truck until I knew what was causing it to chug.
