Well, i think the big problem is the coolant they think they have to use in these motors, it can't take the heat the silicates drop out and the first stop is the oil cooler but the egr cooler is the one that goes out a good flush and ELC coolant would save a lot of trucks that have low miles on them. I think if you a truck has 40-50K on it a coolant flush will save it but more the miles it has on it the greater the chance of plugging a oil cooler so in my book this is the main reason guys pop HG high heat in the temp spread between the EOT-ECT and then the egr cooler goes out then you have white smoke keep driving it then it lets go and start to puke
Mine has 30k on it with a spread of 2-4* did my flush and still have a spread of 2-4* but it took me 12 1/2hrs and 48 gallons of distilled water and 1/2 gallon of Restore and 1/2 gallon Restore+ and a new t/stat. I have had my coolant filter on for over 2 years installed at 17k and yes i had casting sand.
