The device sucks the entire system clean, then a cleaning solution is added and pulled out, and then the new fluid added. While you don't change the screen, everything I've heard says that the screen looks perfect after using this device and then dropping the pan to take a look. The real advantage is that you don't have to drop the pan, so you don't have to mess with the gaskets and have a potential for leaks. It usually costs ~$60-$80 for the service.
