These symptoms are consistent with a sticking idle control motor (less
likely, a sticking throttle valve in the throttle body).  Did the problem
go away for awhile after the idle control motor had been replaced?  First
thing I'd try would be one of the throttle-body-friendly fuel injection
air intake cleaner/sprays.

I believe you may have injector problems. You didn't say how many miles, but
injectors can deliver too much fuel at low speeds and the problem will clear
up as speed increases.
